Vineland-3 adaptive behavior assessment with caregiver interview and record review. Built for LTC and Medicaid Waiver eligibility, guardianship, and service planning.
Biopsychosocial Assessments
Comprehensive person-in-environment evaluations covering history, functioning, supports, and needs. The foundation document for care planning and eligibility decisions.
Specialty Evaluations
Disability and immigration evaluations, adoption and foster home studies. Independent, court-ready, and completed on the timeline your case requires.
How it works
What to expect
Reports are ready within 3 to 5 business days of the interview. Expedited services are available.
Step one
Start with a call or message
We start with a phone call or a message. I learn more about the situation and make sure I can meet your needs before anything is scheduled.
Step two
Schedule and prepare
We set a date and time, I send the intake paperwork, and payment is completed prior to the appointment.
Step three
The comprehensive interview
We meet for the comprehensive interview, including standardized instruments where the referral question calls for them.
Step four
Scoring, report, recommendations
I score, write, and make recommendations. Your report is ready within 3 to 5 business days, with expedited service available.

Who I am
About Stephanie
I am Stephanie A. Smith, LCSW, ASW-G, C-ASWCM. My background spans residential care, child welfare, hospital social work, and behavioral health quality improvement leadership in long-term care. I teach social work as adjunct faculty in BSW and MSW programs.
The Social Work Progressive is my independent practice, based in Colorado. I handle every case personally, from the first call through the final report.
